    Help please

    I need some response to the following diet related questions. You don't have to answer everything and you can answer as briefly as you wish but I'd be extremely grateful if you took the time to do this for me. I can assure you that nobody will be able to find out who answered and in what way. All responses will be kept private.

    Are you willing to make some changes in your diet to lose weight?

    How many hours do you spend doing the following activities:

    Sleeping?

    In very-light activity? (Watching TV, sitting in office, driving)

    In light activity? (light house work, hoovering, cooking, golf, walking)

    In moderate activity? (washing floors, carpentry, aerobics, hiking, tennis, jogging)

    In heavy activity? (moving furniture, construction, boxing, running, rowing)

    Do you cook for yourself?

    What do you normally eat for the following meals:

    Breakfast?

    Lunch?

    Afternoon Snack?

    Dinner?

    Bedtime Snack?

    What is are your favourite foods for the following times, how often do you eat these:

    How much water do you drink in a typical day?

    How much alcohol do you drink in a typical week (1 unit is half a pint of 4% beer, as standard shot (25ml)of 40% spirit, a standard measure (50ml) of sherry or port (20%) there are 1.5 units in a small glass (125ml) of 12% wine)?

    Do you eat when you are emotionally sad, angry, bored, stressed?

    What time do you have dinner?

    Do you exercise regularly?

    Are you having difficulties with your diet/ing?

    Do you crave any foods?

    Where you heavy as a child?

    Do you want to lose weight?

    If so, what is your goal?

    Has your doctor given you any activity or food restrictions that you currently follow or do you avoid certain foods for medical reasons? If yes please explain.
